Proxy Ballot for the Annual Business Meeting

If you cannot attend the Annual Meeting, please print and fill out this Proxy form and have someone bring it to the meeting for your vote to be counted.

To initiate a General Meeting, there must be a quorum of 50% of the membership. The quorum is determined by the number of Voting Members who are present at the meeting plus the number of proxy votes.

To approve any decision, other than the Bylaws, at least 50% of the votes are required.

Changes to the Bylaws require at least 2/3 of the votes by members present.
